Armed Forces Compensation Scheme - Claims Form

The form above should be completed if you want to make a claim under the Armed Forces Compensation Scheme (please ensure you have contacted us for free legal advice first).

You must be serving or have served in HM Forces (including Reserve Forces and Brigade of Gurkhas) and your illness or injury was caused by your service on or after 6 April 2005. There is a seven year time limit for making a claim under the Armed Forces Compensation Scheme.

If you are no longer serving and your illness or injury was caused by your service before 6th April 2005, you may be considered under the War Pensions Scheme.

The AFCS does not prevent you pursuing a personal injury claim for compensation if the MoD has been negligent and a civil claim may lead to a higher award in respect of damages. To get advice on which course of action is the best for you, contact our armed forces team.

Pax insurance

If you have the benefit of PAX insurance you may be entitled to recover a sum of money for your injury under the Personal Accident scheme if your injury falls within the range of injuries outlined in your policy.

You should call the PAX Customer Services helpline on 0800 212 480 or (+44) 20 8662 8126 from overseas and request a copy of the Claim Form to complete.
